Any questions not answered on this website will only be answered after you have been accepted into my practice. This is for everyone's safety. Anyone suspected of trying to get around this policy will be refused service. I have conveniently added my intake form to the bottom of this page for anyone who wishes to become a client. I also will not discuss scheduling until your intake questionnaire is completed and approved.

*List all otc or Rx medications and supplements used, especially aspirin or prescribed blood thinners.
*List any medical conditions including prior surgeries, skin conditions such as psoriasis, eczema, toenail fungus, etc.
*Please read this informed consent paragraph before completing the form.	I understand that the bodywork I receive is provided for the basic purpose of relaxation and relief of muscular tension. If I experience any pain or discomfort during this session, I will immediately inform the practitioner so that the pressure and/or strokes may be adjusted to my level of comfort. I further understand that bodywork should not be construed as a substitute for medical examination, diagnosis, or treatment and that I should see a physician, chiropractor, psychologist, or other qualified medical specialist for any mental or physical ailment that I am aware of. I understand that bodywork practitioners are not qualified to perform spinal or skeletal adjustments. Because bodywork should not be performed under certain medical conditions, I affirm that I have stated all my known medical conditions, and answered all questions honestly. I agree to keep the practitioner updated as to any changes in my medical profile and understand that there shall be no liability on the practitioner part should I forget to do so. It is also understood that the therapist will be working on my whole body including muscles of my lower abdomen and upper and inner thighs. If I have any issues or limitations of contact with these areas or nearby areas, I will make a note of it in the space provided below so the therapist can take extra care not to have any even accidental contact with such sensitive areas. I understand and agree that by writing "none" I grant permission to work freely and come into contact with all areas whether intentional or incidental to the nature of work on surrounding areas. I further agree that by not making such note, I have consented to this contact with no expectation or promise of such. I further acknowledge and accept that the session fee and any voluntary tips I choose to add are for the bodywork only and will not encourage any type of sexual service or contact. I agree that if I desire sexual (vaginal, oral, anal) intercourse I will seek it elsewhere with a different consenting adult and not from my bodywork specialist. If, in any way, I mistreat the specialist so that she feels compelled to warn others of my abusive behavior, I give my permission in advance for her to do so.
